This form needs to be submitted for new workshops booked at MCO. If you have taught before you will only need to fill out the required fields.

I understand that there is a 60/40 split of the cost of the workshop of money collected. MCO carries the liabilities of rent, utilities, insurance, cleaning and handling of funds

I understand my workshop will be listed on the MCO calendar and will be promoted.

I understand MCO cannot guarantee to fill a class. Part of the responsibility rests on the teacher.
